Description
Spartanburg Regional Healthcare System is seeking a full-time Certified Nurse Practitioner or Physician Assistant to join our Oncology Survivorship Program. The APP will work collaboratively with a multidisciplinary team to provide comprehensive follow-up care, symptom management, and wellness support for patients transitioning from active cancer treatment to survivorship.
Position Highlights:
- Monday-Friday 8a-5p with occasional event support after clinic
- No weekends, holidays or call
- Outpatient (Inpatient to provide support to Hem Onc as needed)
- Primary Location: Spartanburg & Pelham (Union and Gaffney as needed)
Responsibilities:
- Survivorship Visits & Clinical Support
- Comprehensive Survivorship Assessment
- Risk-Stratified follow up and shared care
- Psychosocial distress screening and response
- Survivorship Care Plans (SCPs)
- Clinical management and coordination
- Program leadership and accreditation
- Education and communication
- Outcomes and quality
- Documentation and compliance
Minimum Requirements:
- Minimum of 2 years of advanced practice provider experience in Oncology
- Strong clinical assessment and decision-making skills
- Excellent communication and interpersonal abilities
- Commitment to patient-centered care and health equity
